Acknowledgment and Assumption of Risk

I, the undersigned, wish to participate in the martial arts class offered by Parkstone Karate, which includes training in Goju Ryu Karate along with other associated styles of combat and self protection as advertised. I understand that these activities involve rigorous physical exertion, close contact with other participants, and potential physical contact with equipment and the ground. These activities carry inherent risks that cannot be eliminated regardless of the care taken to avoid injuries.

The risks include, but are not limited to, bruises, strains, sprains, fractures, dislocations, and, in rare instances, more serious injuries or conditions such as concussions or other impacts, injuries from falls, and in extreme cases, paralysis or death. I also acknowledge the risk of emotional and psychological injuries or stress and the possibility of experiencing fear and the risk of exposure to communicable diseases.

I understand that while Parkstone Karate and its instructors strive to maintain a high level of safety and to teach techniques and methods to reduce the risk of injury, it is not possible to completely eliminate these risks. Participation in these activities is voluntary, and I am participating with full knowledge of the inherent risks involved.

By signing below, I expressly accept and assume all of the risks involved in participating in martial arts classes at Parkstone Karate. This assumption of risk includes any risks that may arise from the negligence of Parkstone Karate, its owners, affiliates, instructors, employees, or other participants, or from dangerous or defective equipment or property owned, maintained, or controlled by them.

In consideration of being permitted to participate in the martial arts classes, I, for myself, my heirs, personal representatives, or assigns, do hereby release, waive, discharge, and covenant not to pursue Parkstone Karate, its owners, directors, agents, instructors, and employees from liability from any and all claims resulting in personal injury, accidents, illnesses (including death), and property loss arising from, but not limited to, participation in martial arts activities.

 

Acknowledgment of Understanding

I have read this assumption of risk form and fully understand its terms. I acknowledge that I am signing this agreement freely and voluntarily, and intend my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.

Junior Karate Classes

 

To the parent/guardian,

We provide junior karate classes which you’ve expressed an interest in your child joining. Accordingly, this is a brief introduction to myself and the upcoming session. There is also an assumption of risk form that is required signed and completed prior to the class commencing.

What’s being taught?

Your child will be learning Goju Ryu Karate. To be more specific, this includes a combination of striking (punching), kicks, elbows, and knees, blended into a co-ordinated syllabus comprising of both individual and partner practice. 

The content will of course be appropriately modified to the age range in question, and to the level of experience. 

Who’s teaching?

The session will be led by Andy and Megan Barber, qualified martial arts instructors. The class is provided by Parkstone Karate. 

The instructors are; 

  • Black belts in Goju Ryu karate – Sensei Andy is a 5th Dan, Sensei Megan a 3rd Dan.
  • Between them they have over 40 years teaching experience. 
  • They are both hold enhanced DBS checks and Safeguarding qualifications.
  • Both are first aid trained. 

The instructors may also be assisted by senior black belt students from the school.

JUNIOR STUDENT ASSUMPTION OF RISK FORM

 

Martial arts – as with all sports – carry an inherent risk of injury or harm and it is essential that you are fully aware of these dangers before permitting your child the opportunity to participate in our class. Please read carefully the following document and sign only if happy to proceed.

Martial Arts carry inherent risks. Whilst your instructor will always do what is possible to minimise these risks and mitigate the possibility of harm occurring, there are certain dangers that are unavoidable.

Because of the nature of martial arts – and unarmed combat training – there is a managed exposure to potential risks throughout training with Parkstone Karate. These include, but are not limited to, Slips, Trips, Sprains, Falls, Cuts, Abrasions, Contusions, Swelling and in some more uncommon cases, breaks and other injuries. Whilst the club’s safety record is exemplary it’s important to us that you understand the nature of what your child is about to participate in, and that you are happy to assume all risks having been made aware in advance of what these might be.

We train in Goju Ryu karate There is an element of physicality within all of our classes, and general physical activity is part of the training.

All participants have the right to stop training at any point should they not feel comfortable performing any set technique or exercise, and they are under no pressure to complete any drill, technique or exercise if they do not wish to. The instructor will make clear before the session all rules and important safety guidelines. We ask that you remind your child of this.

If you do give permission for your child to participate within the class we ask that you take a moment to consider the nature of self defence training and the techniques this might entail. Injuries are generally uncommon, but we cannot offer any guarantees. Your child’s co-operation with fellow students, and their attention to the instructor is essential and we would ask you to, as condition of their participation, ensure they listen to instructions carefully to help avoid injury.

You are welcome and encouraged to speak to the instructor if you’re not completely at ease with the risks being assumed, or not completely confident about what our classes entail.

Please ensure the instructor is aware of any medical conditions or injuries relevant to your child BEFORE the class commences.
